Phyllis Ann Chapman, 67, Linton passed away at 6 a.m. on Thursday at her residence.
Born May 23, 1939 in Dugger, she was the daughter of William and Mary (Leigh) Cliver.
A homemaker, she was a member of the Linton Church of Christ and Order of Eastern Star in Linton.
Survivors include: two daughters, Treina (Dennis) Cullison of Worthington and Mary Beth (Jeff) Jenson of Terre Haute; one son, Jerry R. Chapman, Jr. of Dugger; two sisters, Patty Mabnuel and Freida Boyd, both of Dugger; five brothers, Tom and Charles Cliver of Linton, Danny Cliver of Mooresville, Bobby Cliver of Dugger, and Jerry Cliver of Florida; six grandchildren, Gerren and Brianna Cullison, Kaleb and Kristin Newkirk and Darrion and Tyla Chapman; one great-granddaughter, Abigail Newkirk.
She was preceded in death by her parents; her husband, Jerry R. Chapman, Sr.; one sister, Rosemary Smith; and two brothers, John and Bill Cliver.
Memorial service will be at 8 p.m. on Wednesday at Welch & Cornett Funeral Home, Linton Chapel with Jerry Carmichael officiating. A private burial will be Thursday at Fairview Cemetery in Linton.
Visitation will be held at 5 p.m. on Wednesday until the hour of service at the funeral home.
